Mycobacterium marinum: A new threat in German freshwater environments.
In Germany, several clusters of Mycobacterium marinum infections have been identified in the past decade. The bacterium is mainly associated with contact to fish and fish products, but the number of reported cases of M. marinum infections is increasing. In the last 2 decades, in Germany, a steady increase of new cases of M. marinum infections has been documented. In this study, we report on an outbreak of M. marinum infection in a German university aquaculture facility, which is a high-risk environment due to water contact with fish. During the summer of 2014, six cases of M. marinum infection were detected in seven captive fish. Transmission to two people occurred after direct contact to affected fish. Five out of six animals were detected with subcutaneous nodules and skin ulcerations. Histopathological analysis demonstrated subcutaneous and superficial granulomatous inflammation with necrosis and collagen fibrosis. M. marinum was detected by cultivation and genotyped. In addition, mycobacteria belonging to the M. marinum complex were found by DNA-PCR and sequencing. In vitro, M. marinum is able to penetrate and degrade artificial coral material. Submerged artificial coral material that is used to feed fish within aquaculture facilities may therefore be an ideal substrate for mycobacteria, including M. marinum. This article reports the first cluster of M. marinum infections associated with contact to captive fish. The number of M. marinum infections in aquatic animal facilities is increasing. Submerged artificial coral material used as feed for captive fish is a potential source for M. marinum infections in fish farm environments. "You okay?" "Yeah, yeah, it's just..."
